Drug Use Evaluation study on Vancomycin





Abstract
A retrospective drug use evaluation(DUE) study was conducted on vancomycin by reviewing patients charts for adults. "Criteria for Drug Use Evaluation" established by American Society of Hospital Pharmacists was reviewed and revised in DUE process. Total 39 charts of adult inpatients who were treated with vancomycin from Jan. 1. 1991 to July 31. 1991 were retrospectively reviewed.
As a result, 22 cases(56%) were met with the criteria for the justification of use, while remaining 17 cases(44%) were deviated from the criteria. In Analysing process indicators of criteria including body temperature monitoring, WBC monitoring, appropriate initial dosage were well documented showing the accepted levels above 85%, while the duration of therapy and the pretreatment SCr were documented with slightly low accepted level(71%, 68%). In outcome analysis, blood cultures after discontinuing drug were rarely documented with accepted level of 42%.
